Rare Presentation of Marine-Lenhart Syndrome

چکیده
The combination of autoimmune Graves’ disease with a solitary hot thyroid nodule suppressing the remainder of the thyroid gland is rare. We report a case of Graves’ disease associated with a single hyperfunctioning nodule with suppression of the remaining gland. A 45 year old male patient with past medical history of autoimmune Graves’ disease with severe ophthalmopathy was referred to endocrine clinic. His symptoms started with dry eyes several years ago without associated hypermetabolic symptoms. Further investigation revealed hyperthyroidism; he was started on methimazole. Thyroid stimulating immunoglobulin was 484 %( reference: <140%, Method: immunoassay). Thyroid scan with I-123 sodium iodide showed uptake of 28.3 percent at 24 hours, and showed a large area of intense activity mid and lower left thyroid lobe with suppression of uptake in the rest of the thyroid gland. Thyroid ultrasound showed a heterogeneous well-defined 1.3x1.0x1.0 cm left mid lower pole nodule. These findings are suggestive of a rare variant of Marine-Lenhart syndrome. Graves’ hyperthyroidism is thought to be caused by autoantibodies stimulating the TSH receptors. Thyroid nodules associated with Graves’ disease have been reported with the incidence between 25 to 30%, most of which are cold nodules. The incidence of Graves’ disease accompanied by hot nodules ranges from 2.7 to 4.1%.These rare cases of Graves’ disease with accompanying functioning thyroid nodules are known as Marine-Lenhart syndrome. Most reported cases of Marine-Lenhart syndrome are Graves’ disease with multiple hyperfunctioning nodules. Our case represents an unusual variety in which Graves’ disease with a hyperfunctioning solitary nodule was associated with suppression of the remaining gland. Acquired mutations resulting in constitutively activated TSH receptors and the presence of anti TSH receptor autoantibodies might be responsible for the development of Marine-Lenhart syndrome.

Coexistence of Graves' Disease and Toxic Adenoma: a Rare Presentation of Marine-lenhart Syndrome.
Both Graves' disease and Toxic Nodules cause thyrotoxicosis, albeit by different mechanisms. Their coexistence is called Marine-Lenhart syndrome, the prevalence of which has been reported 2.7-4.1%. In many cases with Marine Lenhart syndrome Graves' disease is accompanied by multiple hyperfunctioning nodules, although it is accompanied by a solitary hyperfunctioning nodule in rare cases. We here...
متن کاملA Case of simultaneous occurrence of Marine – Lenhart syndrome and a papillary thyroid microcarcinoma
BACKGROUND Marine-Lenhart syndrome is defined as the co-occurrence of Graves' disease and functional nodules. The vast majority of autonomous adenomas are benign, whereas functional thyroid carcinomas are considered to be rare. Here, we describe a case of simultaneous occurrence of Marine-Lenhart syndrome and a papillary microcarcinoma embedded in a functional nodule. CASE PRESENTATION A 55 y...
متن کاملMarine-Lenhart syndrome with papillary thyroid carcinoma
Graves' disease with accompanying functioning nodules is known as Marine-Lenhart syndrome. Autonomously functioning thyroid nodules (AFTNs) also within Graves' thyroid tissue are almost always bening in nature. A 45-year-old man developed hyperthyroidism due to the coexistence of Graves' disease and AFTN. Total thyroidectomy was performed. The hyperfunctioning nodule with centrally hypoactive f...
متن کاملThe oldest case of Marine-Lenhart syndrome?
Thyroid dysfunction is relatively common, with hypothyroidism in women reported to have a prevalence of 4.1 per 1000 population, and 0.8 per 1000 having hyperthyroidism. Unlike hypothyroidism which becomes more prevalent with age, Grave’s disease – autoimmune hyperthyroidism – seems to not do this, with the incidence remaining relatively steady at 1 in 1000 per year between the ages of 35 and 6...
متن کاملSheehan’s syndrome presenting as psychosis: a rare clinical presentation
Sheehan’s syndrome (SS) refers to the occurrence of varying degree of hypopituitarism after parturition (1). It is a rare cause of hypopituitarism in developed countries owing to advances in obstetric care and its frequency is decreasing worldwide. However, it is still frequent in underdeveloped and developing countries. Sheehan’s syndrome is often diagnosed late as it evolves slowly (2,3). Re...
